All Jamboree Participants must complete the new online medical form process, and turn in a medical form that is less than one year old by the end of the Jamboree. The deadline for submitting medical forms to the National Jamboree Medical Team is April 15.
In December you should have received an email from the Jamboree Team with some instructions regarding this medical form. To access it, you will need to login to your summit account at http://summit.scouting.org. Once you login to your dashboard, you will see a link for medical forms at the top of the dashboard.
Not all browsers can access this information. As of now, you must access with Firefox 14+ (Mac or PC) , Google Chrome 21+ (Mac, PC, iOS and/or Android), and/or Internet Explorer 9.0+ (with compatibility mode OFF). These procedures and system requirements will be the same as for your selection of Jamboree activities.
One tip: save your work after each screen you complete on the online medical form. If your browser crashes without saving, you will have to re-enter everything since your last save.
After you have completed the online form, you will have to print a copy. Please be aware that this form is eleven pages. Pages 10 and 11 will have to be completed by an examining health care provider. You will also have to attach a copy of your health insurance card (front and back) to the packet.
After you have completed the form, please make a minimum of two copies. Keep one copy for yourself and bring the other copy to the March 16th Jamboree meeting in Great Falls. You do not have to mail a copy to National; we will submit Part C and a copy of your medical card. You might also want to make an additional copy for your home troop.
